Light Disability support coordination services are designed to assist individuals with disabilities and their families in navigating and maximising the benefits of their NDIS plans. We aim to empower participants by providing them with the necessary information, resources, and guidance to make informed decisions about their supports and services.
We help participants understand their NDIS plan and how to implement the supports outlined in the plan effectively. Light Disability Services provides guidance on service providers, available funding, and how to access and utilise supports.
Light Disability Services assists participants in finding and connecting with suitable service providers who can deliver the required supports. We help participants identify providers in their area, understand their services, and facilitate the engagement process.
Support workers help with household tasks and chores such as cooking, cleaning, laundry, shopping, and maintaining the living environment. The goal is to ensure a safe, comfortable, and well-maintained living space for all participants.
We work with participants to build their capacity and skills in managing their NDIS plans independently, provide training and resources to help participants effectively communicate their needs, set goals, and make informed decisions about their supports.
Light Disability Services acts as a central point of contact between participants, service providers, and other stakeholders involved in the participant's NDIS journey. We coordinate services, ensure seamless delivery of supports, and facilitate collaboration between different providers.
We advocate for participants' rights and interests, ensuring their needs are met within the NDIS framework, assisting in resolving any issues or concerns that may arise during the planning or implementation process.
We help participants monitor their progress, evaluate the effectiveness of their supports, and provide regular reports to the NDIA (National Disability Insurance Agency) regarding the participant's NDIS plan and outcomes.
