Support Coordination
Adon Care's Support Coordination service provides personalized guidance and advocacy for individuals facing challenges or living with disabilities. Our experienced Support Coordinators assess individual needs, create tailored Support Plans, and connect clients with suitable support services and funding options. With a compassionate approach, we empower individuals to achieve greater independence and improve their quality of life, advocating for their rights and preferences throughout the support journey. Adon Care's team continuously monitors progress, conducts reviews, and fosters community engagement to ensure a fulfilling and inclusive future for their clients.
Support Coordination Level 2
Continuing support throughout the duration of your plan, for people with more complex needs who may require additional support to connect with providers.
Specialist Support Coordination Level 3
Higher intensity Support Coordination. Our multidisciplinary qualified team with you and your support network to action your NDIS plan and reach your potential.
Psychosocial Recovery Coach (PRC)
Will provide support to people with psychosocial disability to live a full and contributing life. People will be supported to take more control of their lives and to better manage complex challenges of day to day living.
Social & Community Participation
Adon Care's Social & Community Participation service empowers individuals with disabilities to engage in social activities, build connections, and integrate into their communities. With personalized support, inclusive environments, and various outings, individuals can develop life skills, gain independence, and experience a fulfilling life. The service also offers virtual options, involves family and carers, and fosters a sense of belonging and joy through community engagement. Innovative Community Participation
This activity is designed to support NDIS participants to build skills to actively participate in their community. Through side-by-side practice, the activity involves highly skilled Community Engagement Experts who assist participants to expand their opportunities for community participation and employment.
Group and Centre Based Activities
Group activities are a great way to build strong friendships with associates of similar age and interests. We offered the services for mixed groups for ages between 16 – 65. Our group’s objectives to increase participant’s capacity, establish new friends, and increase social & community participation.
Transport
Adon Care's Transportation Assistance service provides safe, reliable, and personalized transport for individuals, including seniors and those with mobility challenges. The service offers accessibility features in their vehicles and covers medical appointments, social outings, errands, and grocery shopping. With compassionate and flexible support, Adon Care ensures stress-free travel, empowering individuals to experience the freedom of mobility and maintain their independence.
Supported Independent Living Assistance
We offers personalized support and guidance to individuals seeking to live independently while receiving necessary care. They create individualized Independence Plans, foster safe and nurturing environments, and provide daily living support and life skills development. The service promotes social inclusion, coordinates healthcare needs, and involves families and carers. Adon Care empowers individuals to lead self-directed lives, embracing independence and creating a future filled with possibilities.
Respite or short-term Accommodation
The NDIA price guide defines the STA line item as Combined support for self-care, accommodation, food, activities in a centre or group residence for short periods. This line item can be used flexibly over 24 hours to provide accommodation, food & negotiated activities over a short period.
In-Home / Domestic Assistance
Our personalized support and help with household tasks to ensure a clean, comfortable, and organized living space. Services include housekeeping, meal preparation, shopping, personal care, and companionship. With a respectful and caring approach, flexible scheduling, and a focus on supporting independent living, Adon Care's caregivers provide peace of mind for families and create a comfortable home environment that enhances well-being and promotes independence.
Nursing Care
